We look forward to creating some amazing art for you, but honesty is key. Our clients and staff members are following many guidelines and restrictions for the safety of your health and ours. Appointment Request Form

Fill out the following in its entirety and send to: artisanaltattoocalendar@gmail.com or the provided contact information. Include in the email subject line “Tattoo Request” with your full name.

If you do not include all of the required information, then you will not receive a response.

After this Form is Submitted:

We will contact you with further questions within 5-7 business days (excluding off days and holidays).

You will be given an appointment date and will be required to place the $100 deposit at that time. The deposit will go towards the total price of the tattoo. It is required for all appointments and is not refundable. No exceptions.

  • If this form is not filled out correctly in its entirety and/or there are no attached references, then you will not receive a response.

  • If your tattoo request is better suited for another artist, you will be redirected to them.

  • If your tattoo request is better suited for a Walk-In, you will be redirected to do so.

  • Deposits are non-refundable. Cancellations within 24 hours of your appointment will result in the forfeit of your deposit. No shows will result in the forfeit of your deposit. No exceptions.

Gift Cards are not applicable to Shades of Swift or Piercing Appointments.
